Request to Transfer Select Agents and Toxins

The APHIS/CDC Form 2, Request to Transfer Select Agents or Toxin, is used by entities to request authorization from APHIS or CDC to receive or send a Select Agents or toxin.

A registered entity is required by regulation (7 CFR 331.16, 9 CFR 121.16, and 42 CFR 73.16) to file this form with either APHIS or CDC and obtain approval prior to transfer of a Select Agents or toxin. The purpose of this form is to provide a method for the documentation of the transfer of a Select Agents or toxin. The form must be completed for each transfer of Select Agents.
